After you have been admitted and decided to enroll at MVPNS, we will require that you complete certain forms. Completion of these forms enables us to meet state and local requirements, and to ensure the safety and well-being of all of our children. (Be assured that all information submitted to MVPNS is handled in a confidential manner.)

11. Photocopy of current automobile insurance that includes your name, dates of coverage, and bodily injury liability coverage**

* Students currently enrolled at MVPNS and new students entering either the MWF AM or MWF PM class are eligible to attend summer camp.
** These forms, as well as an updated photocopy of your child's immunization record/report, are required by families whose child(ren) will be continuing on at MVPNS for a second or third year.
