After you have been admitted and decide to enroll at MVPNS, we require that you complete certain forms. Completion of these forms enables us to meet state and local requirements, and to ensure the safety and well-being of all of our children. (Be assured that all information submitted to MVPNS is handled in a confidential manner.)

1. New Families - Families with a child enrolling in MVPNS for the first time
2. Current Families - Families who were enrolled in the 2010-2011 school year and are continuing on at MVPNS for a second or third year
3. Returning/Alumni Families - Families with older siblings previously enrolled in MVPNS who are returning to MVPNS with a younger sibling
4. Summer Camp Application - Students currently enrolled at MVPNS and new students entering either MWF AM or MWF PM class are eligible to attend summer camp  |
