• MVPNS Work Day Preference Sheet
• Identification and Emergency Information Form
• Child's Preadmission Health History - Parent's Report
• Consent for Emergency Medical Treatment - Half Sheet: This form will be kept in the classroom in case of an emergency.
• Physician's Report - Child Care Centers: Child's health form, must be completed by child's physician.
• Health Screening Report - Facility Personnel: Work parent/caregiver's health form, must be completed by adult's physician.
• Tuberculosis Screening Documentation for Incoming MVPNS Families: TB test for work parent/caregiver, must be completed between July 1, 2011 and the first day of school in September.
• Consent for Emergency Medical Treatment/Emergency Contact and Release - two-part, two-sided form: Please complete the ENTIRE form (front side AND back), even though the information is duplicated. The form eventually gets divided, with half placed in an emergency backpack and half in an emergency barrel (in case of an earthquake).
• Consent for Security Background Check
• Photocopy of Current Automobile Insurance: Must include name, dates of coverage, and bodily injury liability coverage.
• Photocopy of Current Driver's License
